Ms. Battistoni has been working in the geriatric and health field for over twenty years. As a care manager, her expertise includes:
- Comprehensive assessment of clients’ medical needs, social situation and living environment.
- Assisting clients and their families in understanding the current and future ramifications of client’s condition
- Providing information and support as needed.
- Assisting client in obtaining appropriate physicians, therapists and care providers, and coordinating care within the multiple disciplines necessary.
- Family liaison with physicians and other providers; anticipation of future issues and the interventions which may be needed; setting up and implementing appropriate care plans.
- Patient advocacy with physicians, therapists, hospitals, equipment providers, and insurance agencies; tracking and coordinating medications for client.
- Assisting family and client in management of financial affairs; negotiating interpersonal issues with client, providers, and family members.
- Primary contact for client and providers in emergencies.
- Assisting family members in both approaching issues over client’s situation and obtaining counseling as needed.
Prior to care management Ms. Battistoni worked as a certified, licensed Physician Assistant at Mount Sinai Hospital in New York and before that at Jacobi Hospital in the Bronx. At both hospitals her specialty was inpatient medicine, mostly with an elderly population. Before working in medicine Ms. Battistoni was a geriatric community worker for Selfhelp Community Services, a social work organization in Manhattan.
